The salary range for the KMS Patient Care Coordinator is $16.

The KMS Patient Care Coordinator will work M-F 8:
00am to 5:
00pm out of the Holland office.
Responsibilities of the KMS Patient Care Coordinator include:
Adds new physician and updates current physician information in Consolo including physician license expiration/renewal, address and contact information.
Processes physician orders:
Assisting with getting home health orders signed by physicians and faxed to the appropriate home care agency.
Assisting with faxing physician orders to appropriate primary care physicians (PCPs) Provides administrative support to KMS Palliative Care (PC), including but not limited to:
Tracking PCP consents and approvals Processing advanced directives and updating appropriate spreadsheets Scanning consents and opioid forms Entering referrals and ensuring a prompt response to requests for PC consults Tracking when patients need to be seen Contacting providers to ensure PC referrals are being processed Maintain Vendor/Contact Information in Consolo Medication Reconciliation and Entry Accessing information via VIPR PC Data Entry and report tracking (internal and payers) Preparing and sending medical record requests Performs the tracking and overall facilitation of patient and physician satisfaction surveys.
Acts as a back-up for PC Scheduler and provides lunch coverage for hospice intake coordinator.
Provides administrative support to the Medical Director, physician or nurse practitioner as required.
Assists with KMS Policy and Procedure review and updates as needed Assists with tracking of KMS Customer Concerns, Incident Reports, medication Errors, HIPAA requirements, and other compliance measures as needed.
Assists with tracking and facilitating MIPS requirements of provider team and physician office practices.
Assists the KMS team in delivering the best care possible to those on KMS service.
Qualifications for the KMS Patient Care Coordinator include:
High school diploma or GED; At least one year of general office function experience.
Familiarity with Excel Pharmacy Tech Certification (preferred but not required) Able to work as part of an interdisciplinary team.
Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities.
Strong written and verbal communications skills and public relations skills.
Ability to exercise sound judgment and critical thinking with regards to operational efficiency and productivity, including soliciting and reviewing quotes.
Capable of utilizing all related administrative tools; computer information systems including MS Office, phone systems, office equipment and support tools.
